Alliance Health Group's Cedar Hills Center for Nursing and Rehabilitation in Clemmons, NC seeks an LPN Charge Nurse to lead and deliver direct patient care in a skilled nursing setting. You will supervise CNAs, participate in care planning, and ensure safety, quality, and documentation standards.

The ideal candidate holds an active NC LPN license, CPR/First Aid, strong clinical judgment, and excellent communication.

Qualifications

• Active Licensed Practical Nurse (LPN) license in North Carolina.
• Current CPR and First Aid certification.
• Strong clinical judgment and ability to respond to changes in patient condition.
• Leadership ability with strong communication and teamwork skills.
• Compassionate, dependable, and resident-focused approach to care.
• Ability to work effectively in a fast-paced skilled nursing environment.

Responsibilities

• Administer medications and monitor residents for therapeutic response and side effects.
• Perform ongoing assessment of resident condition, including vital signs and clinical status changes.
• Provide wound care, catheter care, and specimen collection as needed.
• Oversee and support CNAs to ensure consistent, high-quality resident care.
• Communicate changes in resident condition to the RN and interdisciplinary care team.
• Participate in care planning, updates, and resident care coordination.
• Maintain accurate, timely, and compliant clinical documentation.
• Promote resident safety, comfort, dignity, and overall well-being.
• Ensure adherence to facility policies, procedures, and regulatory standards.
