Job summary

Four County Health & Rehab in Georgia is seeking a Licensed Practical Nurse for full-time Day shift. You will administer medications, monitor residents, document care, and supervise CNAs as part of an interdisciplinary team.

Starting pay ranges from $25.75 to $29 per hour with shift differentials evenings, nights and weekends. Benefits include health/dental/vision, 401(k) with match, PTO, holidays, life and disability insurance, and a supportive, mission-driven workplace.

Qualifications

• Active LPN license (GA or eligible).
• Strong clinical judgment and critical thinking.
• Excellent communication and teamwork abilities.

• Prior long-term care experience preferred, but not required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care according to care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation.
• Ensure compliance with clinical policies and regulatory standards.

Your Impact as an LPN

As a Licensed Practical Nurse, you play a critical role in delivering high-quality clinical care and ensuring the well‑being of our patients. You will work closely with RNs, CNAs, and the interdisciplinary team to provide compassionate, person‑centered care.
