Job summary

Athens Health and Rehabilitation is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care for residents in a home-like setting. You will work closely with RNs and the interdisciplinary care team to support rehabilitation goals, ensuring comfort, safety, and dignity for residents throughout their stay.

Qualifications

• Current and valid Licensed Practical Nurse (LPN) license.
• Strong clinical skills, including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.
• Compassionate, patient-focused approach to care.
• BLS/CPR certification required.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor, document, and report changes in residents’ health status, including vital signs.
• Assist residents with activities of daily living (ADLs) as needed.
• Collaborate with rehabilitation team members to support recovery goals.
• Maintain accurate and timely documentation in compliance with facility policies and regulations.
• Provide emotional support and education to residents and families.
• Ensure a clean, safe environment by following infection control protocols.
