Job summary

South Health and Rehabilitation LLC in Birmingham, AL is seeking a dedicated LPN to join our long-term care and rehabilitation team. You will provide direct nursing care, administer medications, and support rehabilitation goals in a patient-centered setting.

The role requires a current LPN license, BLS/CPR, strong clinical skills, and excellent communication. You will collaborate with RNs, physicians and therapists to ensure high standards of care and resident well-being.

Qualifications

• Current and valid LPN license.
• BLS/CPR certification required.
• Strong clinical skills including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor and document residents’ health status, including vital signs, and report changes to the supervising RN or physician.
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed.
• Collaborate with the rehabilitation team to support residents’ recovery goals, including therapy plans.
• Maintain accurate and timely documentation in accordance with facility policies and state/federal regulations.
• Provide emotional support and education to residents and their families.
• Ensure a safe and clean environment, adhering to infection control protocols.
