Nonotuck Resource Associates, Inc. is seeking a Licensed Practical Nurse (LPN) to provide case management for expanding services, focusing on intake and assessments for new referrals and developing comprehensive home-based care plans.

The role emphasizes nursing assessment, care planning, and collaboration with caregivers and families. You will work in a flexible, autonomous capacity across nearby offices with a hybrid office/work-from-home model, driving to member homes as needed and

Qualifications

• Current MA Licensed Practical Nurse (LPN) license required.
• Strong computer and organizational planning skills.
• Experience with elders or chronic conditions beneficial.

Responsibilities

• Handle intake and assessments for new referrals and required follow-ups.
• Provide supervision, training, and evaluation of caregivers.
• Develop and maintain individualized Plans of Care.
• Conduct on-site visits and monitor health status.
• Attend meetings with caregivers, families and staff.
• Document changes and progress in plans and notes.
• Coordinate discharge planning with healthcare teams.
• Provide ongoing education to members and caregivers.
• Participate in on-call system and weekend coverage.
• Drive to homes of people served and collaborate with team.
