Qualifications

Responsibilities

• Conduct intake and assessments for new referrals.
• Develop and maintain Individualized Plans of Care.
• Provide supervision and training for caregivers.
• Monitor health status of members and perform on-site visits.
• Participate in an on-call system when required.
