Job summary

Valley Health System is seeking an LPN Case Manager to support RN Case Managers in discharge planning and post-acute care coordination across Virginia, West Virginia, and Maryland. You will conduct pre-screens, admissions, and Medicaid insurance authorizations for skilled nursing and transitional care beds, and assist with Level 2 assessments while communicating with the care team.

• Two years bedside experience required.
• Licensed as an LPN in the State of Virginia required; BLS certification (AHA Healthcare Provider) required.

Responsibilities

• Assist RN Case Managers in discharge planning and care coordination.
• Complete clinical assessments and pre-screens for post-acute services under Medicaid (VA, WV, MD).
• Process admissions and insurance authorizations for skilled nursing/transition beds.
• Maintain communication with care management on approval/denial of assessments and coordinate community resources.

Job Description

The LPN Case Manager will assist the RN Case Managers in their daily role and processes to organize and facilitate thorough discharge plans for the patients. The LPN Case Manager will be responsible for completing any clinical assessment for patients requiring post-acute services under State Medicaid programs including, but not limited to, Virginia, West Virginia, and Maryland. The LPN Case Manager will be responsible for completing pre-screens, admissions, and insurance authorizations for Valley Health’s skilled nursing facility and transitional care beds. Assisting Case Management staff with completion of Level 2 (MIMR) assessment. Interviewing patients, family members, and/or caretakers as needed to complete assessments. Maintaining open communication with care management team regarding approval/denial of clinical assessments. Maintaining proficiency in electronic submission programs and awareness of current policies/trends related to clinical assessments for Medicaid services. Assists with the arrangement and follow-up with community resources. LPN Case Manager will assist in providing a seamless transition utilizing the entire continuum of care and helping to facilitate patients movement to the next level of care.

Extra duties include, but not limited:
