Job summary

Memorial Community Hospital & Health System in Blair, NE is seeking a compassionate RN or LPN for our Rural Health Clinic. You will perform telephone, electronic, and in-person triage, assess patients, and coordinate timely care.

This role emphasizes critical thinking, patient education, and collaboration with providers to ensure right care at the right time. Experience with Epic or other EHR is preferred; BLS/ACLS per requirements.

Qualifications

• Graduate of an accredited Nursing Program.
• Current RN or LPN license in Nebraska, Iowa, or compact state.

Responsibilities

• Conduct telephone, electronic, and in-person triage assessments.
• Recognize routine, urgent, and emergent patient needs and determine level of care.
• Provide home care guidance, schedule appointments, coordinate same-day visits, and facilitate emergency care when necessary.
• Manage clinic communications: telephone encounters, patient portal messages, refill requests, provider and nursing messages, test results, and care coordination.
• Review and process medication refill requests according to protocols.
• Assess walk-in patients with acute concerns and obtain vital signs and pertinent information.
• Identify signs requiring immediate intervention and activate emergency response when indicated.
• Communicate test results and recommendations to patients clearly.
• Assist patients navigating MCH services, referrals, and community resources.
• Maintain accurate EHR documentation and collaborate with teams for efficient care.
