Live Well Homecare is seeking a Licensed Practical Nurse to serve as a patient navigator, guiding new patients and delivering education to ensure continuity of care. The role collaborates with healthcare providers to support diagnosis and treatment, assisting patients through effective communication and coordinated services.

Qualifications

• Minimum of 2 years LPN experience.
• Cardiovascular and echocardiography experience preferred.
• Graduate from a Specialty Training Program in Nursing.

Responsibilities

• Facilitate patient entry into the program and evaluate their needs primarily via phone or documentation review.
• Coordinate care plans, serve as a communication link between patients and providers, and assist with scheduling and support activities.
• Provide patient and family education, maintain clinical records, and monitor equipment and environment for safety and efficiency.
• Develop screening tools, investigate patient concerns, and assist in medical record reviews.
• Support provider procedures and ensure timely follow-up and surveillance of patients.
