Job summary

Passion Care is seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care across diverse healthcare settings. You will perform patient assessments, administer medications, and support patient safety with vital signs monitoring and physical exams.

You will collaborate with interdisciplinary teams to ensure HIPAA-compliant, NCQA-aligned care, maintaining accurate documentation and contributing to discharge planning and case management when needed.

Qualifications

• Valid LPN license in good standing is required.
• Strong knowledge of medical terminology and anatomy/physiology.
• Ability to work independently while collaborating with multidisciplinary teams.
• Excellent documentation skills ensuring HIPAA and NCQA compliance.

Responsibilities

• Conduct patient assessments including vital signs and exams.
• Administer medications and injections as prescribed.
• Document all patient interactions accurately.
• Assist with diagnostic evaluations and specimen collection.
• Provide care for patients with complex needs (Alzheimer's, dementia, trauma, infections).
• Support discharge planning and case management for transitions of care.
• Educate patients on health topics such as nutrition and chronic disease management.
• Maintain sterile processing standards and infection control practices.
