LCMC Health is seeking a licensed practical nurse (LPN) Patient Navigator for University Medical Center’s Women’s Health Clinic in New Orleans, LA. The role focuses on advocacy, care coordination, education, and seamless patient experiences across the care continuum.

As part of a multidisciplinary team, you will coordinate communication among patients, families, and providers, support treatment planning, and help remove barriers to care while fostering compassionate, patient-centered outcomes.

Competenze

• Graduate of an accredited Practical Nursing program.
• Current LPN license from state board.
• BLS certification for Healthcare Providers.

Mansioni

• Coordinate care across inpatient and outpatient settings with clinicians and support services.
• Advocate for patients and families, guiding them through treatment decisions and system navigation.
• Educate patients and families about diagnoses, treatments, side effects, and resources.
• Support quality improvement, data collection, and policy development within the program.
