Job summary

Qualifications

• Nursing diploma (associate's or bachelor's in nursing).
• Active LPN license in good standing.
• 1 year of clinical post-acute care experience preferred.
• Prior case management, utilization review, and discharge planning preferred.

Responsibilities

• Demonstrate efficient use of computer systems for data entry and retrieval.
• Serve as liaison to external case managers, family, physicians, and resources.
• Train and educate patients, families, and care teams.
• Implement standards of practice for care management and coordination.
• Exhibit excellent customer service and a professional attitude toward patients.
• Maintain confidentiality and adhere to policies and compliance training.
• Communicate effectively within an interdisciplinary team.
