Job summary

Life Care Centers of America seeks an LPN Unit Care Coordinator to supervise and coordinate patient care on their unit, collaborating with patients, families, and nursing staff to ensure compliance with laws and Life Care standards.

The role requires an active LPN license, and CPR certification. Prior geriatric nursing experience is preferred; orientation includes CPR updates as needed.

Qualifications

• Attended an accredited LPN program.
• Must maintain an active LPN license in good standing.
• One year geriatric nursing experience preferred.
• CPR certification upon hire or during orientation and kept current.

Responsibilities

• Direct the daily functions of unit nurses and CNAs to provide leadership on the floor.
• Chart appropriately, accurately, and in a timely manner.
• Provide, manage, and coordinate patient care and services to support patients' physical, mental, and psychosocial well-being.
• Accurately prepare and administer medication as ordered by a physician.
• Coordinate patient care plans and services with RN.
• Demonstrate professional conduct and maintain confidentiality.
