Qualifications

• Currently licensed/registered in applicable State; must maintain an active LPN license throughout employment.
• One year of clinical experience in post-acute care setting preferred.

Responsibilities

• Generate written communication that is clear, concise, and well-organized.
• Serve as liaison to external case managers, family, physicians, and community resources.
• Train and educate patients, families, associates, and other providers of care.
• Implement standards of practice for care management and coordination of care.
• Exhibit excellent customer service and a positive attitude towards patients.
• Assist in evacuation of patients and maintain dependability in attendance.
• Communicate and function on an interdisciplinary team.
• Understand and follow company policies including harassment and compliance procedures.
