Qualifications

• Excellent organizational skills and be efficient in prioritizing and managing time
• Knowledgeable of laws, regulations, and Life Care standards governing post-acute care
• Maintain confidentiality and follow company policies including Code of Conduct

Responsibilities

• Operate computer systems to enter and retrieve data
• Act as liaison to external case managers, family, physicians, and community resources
• Educate patients, families, and care providers
• Implement care management standards and coordination objectives

• Assist in patient evacuation when required
