Infinite Medical P.C. is seeking a responsible Licensed Practical Nurse to review patient charts and ensure records accurately reflect conditions and treatment needs for residents in long-term care settings.

You will conduct comprehensive assessments, support care coordination, and collaborate with interdisciplinary teams to optimize outcomes while monitoring progress and updating EMR documentation as needed. This role emphasizes compassionate care and adherence to clinical standards.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute care or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management services to ensure continuity of care and appropriate utilization of resources.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Overseeing patient EMR to ensure that patients receive proper care.
• Complete reports to reduce patient gaps and hospitalizations.
