Ascend Medical is seeking a Care Coordinator - Licensed Practical Nurse (LPN) to support a healthcare provider across Senior Care Communities. You will document encounters in Athena EMR, coordinate care, and maintain clear communication with facility staff and families.

Qualifications

• 2+ years nursing experience in a primary care setting, preferably geriatric, with current license/registration.

Responsibilities

• Act as primary scribe for the provider during patient visits, documenting in Athena EMR in real-time.
• Serve as the main communications hub between provider and facility staff.
• Coordinate with family members/POA to relay important updates and treatment plans.
• Manage the provider's daily schedule and ensure follow-up visits and referrals are arranged.
• Assist in patient care coordination, ensuring forms and consent are ready and records are up to date.
• Provide clinical support during exams and record vitals as applicable.
• Perform other duties as assigned to support the healthcare team.
