Infinite Medical P.C. seeks a responsible Licensed Practical Nurse to assist with reviewing patient charts and ensuring records reflect conditions and treatment needs.

You will perform comprehensive assessments, collaborate with interdisciplinary teams, and help prevent gaps in care for residents in skilled nursing and long‑term care settings. LPN license required with 2+ years acute or managed care preferred.

Qualifications

• LPN license required.
• Minimum 2 years in acute or managed care preferred.
• Proficient in patient assessments and vital sign monitoring.
• Excellent communication and collaboration with healthcare professionals.
• Experience with geriatric behavioral health preferred.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management to ensure continuity of care and optimal resource use.
• Monitor and evaluate patient progress; adjust care plans as needed.
• Collaborate with interdisciplinary teams to optimize outcomes.
• Oversee EMR to ensure proper patient care and complete reports.
