The candidate should have an LPN license, with a preference for at least 6 months of direct patient care in acute or managed care settings, such as hospitals or nursing homes. Strong communication and EMR skills are essential.

Qualifications

• LPN license is required.
• Minimum 6 months of direct patient care in acute or managed care settings preferred (hospital, nursing home, or home care).
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills for collaborating with healthcare professionals.
• Experience with geriatric behavioral health is a plus.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management to ensure continuity of care and appropriate resource use.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Oversee patient EMR to ensure proper care.
• Complete reports to reduce gaps and hospitalizations.
