Job summary

The Wright Center for Community Health is seeking a Licensed Practical Nurse (LPN) Care Coordinator to join a mission-driven outpatient team. You will perform care coordination via telephone, support patient management, and work with a care team to ensure continuity of care.

Responsibilities include coordinating care for high-risk patients, arranging follow-ups, and handling medications and test results while maintaining HIPAA confidentiality. Rotation includes evenings, weekends, and holidays.

Qualifications

• Active LPN license required.
• BLS certification required.
• Experience in a health center setting is a plus.
• EOS model experience preferred.

Responsibilities

• Oversee care for patients transitioning from healthcare facilities with follow-up calls and care coordination.
• Coordinate care for 30+ high-risk patients using risk stratification tools.
• Arrange follow-up visits and manage patient data on symptoms, medications, and support.
• Handle clinical tasks including drawing up medications and monitoring devices; ensure HIPAA compliance.
• Manage incoming calls, create orders, return test results, handle refusals, schedule appointments, and coordinate outside facility referrals.
