Job summary

The Wright Center for Community Health is seeking a licensed practical nurse (LPN) to join our outpatient care coordination team. You will support patient transitions from facilities, perform timely follow-ups, and coordinate with primary care providers to ensure seamless care management.

Responsibilities include managing data on symptoms and medications, coordinating follow-up visits, and handling calls, test results, and referrals. BLS and PA LPN licensure are required.

Qualifications

• Active LPN license in Pennsylvania is required.
• BLS certification required.
• Experience with electronic health records (Medent a plus).
• Ability to work as part of a care team.
• Commitment to process improvement and quality.

Responsibilities

• Oversee care for patients transitioning from healthcare facilities, including follow-up calls and coordination with Primary Care Provider-Team.
• Coordinate care for at least 30 high-risk and rising-risk patients using risk stratification tools and registries.
• Arrange follow-up visits and manage patient data on symptoms, medications, and support.
• Handle clinical tasks including drawing up medications for clinicians and monitoring devices while maintaining HIPAA confidentiality.
• Answer incoming calls, create orders, deliver test results via phone, process medication refills, schedule appointments, and coordinate outside specialty appointments.
