Infinite Medical P.C. is seeking a dedicated LPN Care Coordinator to join our team in Hollywood, FL. You will review patient charts and liaise with Clinical departments and providers to identify irregularities as part of chronic care management.

The role emphasizes assessment, care planning, and collaboration with interdisciplinary teams within a skilled nursing and long-term care setting. LPN licensure is required, with preference for long-term care and behavioral health experience.

Qualificações

• Experience in long-term care is preferred.
• Experience in behavioral health is preferred.
• LPN license required to practice as coordinator.

Responsabilidades

• Provide assessment and care management services, including administration of validated rating scales.
• Initiate behavioral health care planning for behavioral or psychiatric health problems.
• Revise and modify care plans for patients not progressing or whose status changes.
• Brief psychosocial interventions as needed.
• Engage in ongoing collaboration with the billing practitioner.
• Maintain the registry/tracking sheets.
• Consult with the psychiatric consultant and maintain continuous patient relationships.
• Foster collaborative, integrated relationships with the rest of the care team and conduct interdisciplinary care plan meetings.
