Job summary

Infinite Medical P.C. in Hollywood, FL is seeking a Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within its Collaborative Care Model program.

This is an in-office, non-face-to-face care management role supporting residents in skilled nursing and long-term care facilities. You will review records, perform behavioral health screenings, develop care plans, coordinate services, and provide team-based care under clinical protocols.

Qualifications

• Active and unrestricted LPN license.
• Graduate of an accredited practical nursing program.
• Strong clinical documentation and medical-record review skills.
• Ability to recognize and appropriately upscale behavioral health, medication, and patient-safety concerns.
• Strong written, verbal, organizational, and time-management skills.
• Ability to manage multiple patients, deadlines, and follow-up responsibilities in a structured, high-volume environment.
• Ability to collaborate effectively with providers, psychiatric consultants, facility staff, and clinical leadership.

Responsibilities

• Review patient medical records, diagnoses, medications, behavioral health documentation, and provider notes.
• Document behavioral health screening and monitoring tools (PHQ-9, GAD-7, MoCA, BIMS).
• Monitor patients' behavioral health symptoms and treatment response.
• Maintain ongoing non-face-to-face contact with patients, authorized representatives, and facility staff.
• Develop and update individualized behavioral health care plans with the care team.
• Coordinate behavioral health services and follow-up recommendations.
• Communicate clinical findings and care gaps to providers and leadership.
