Infinite Medical P.C. in Hollywood, FL is hiring a Licensed Practical Nurse (LPN) for an LPN Care Coordinator position in a BHI program. You will review patient charts, assess needs, and coordinate care with a multidisciplinary team.

The role is full-time, in-office, with a 9:00 am–5:30 pm schedule and a salary of $30.75–$33.00 per hour. LPN license required; 2+ years in acute or managed care preferred; strong communication and geriatric behavioral health experience is desirable.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute care or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.

Responsibilities

• Conduct comprehensive assessments of patients’ medical, social, and emotional needs.
• Provide case management services to ensure continuity of care and appropriate utilization of resources.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Overseeing patient EMR to ensure that patients receive proper care.
• Complete reports to reduce patient gaps and hospitalizations.
