Job summary

Carefree Living in Burnsville, MN is seeking a Licensed Practical Nurse (LPN) Care Coordinator to join our dedicated assisted living team. You will work in a resident-centered environment with a supportive interdisciplinary group to deliver high-quality care.

As the LPN Care Coordinator, you will help residents reach and maintain independence, participate in care planning, administer medications, mentor staff, and ensure compliance with policies while upholding confidentiality and compassionate

Qualifications

• Active Minnesota LPN license.
• Excellent communication with residents, families, and staff.
• Experience in Assisted Living preferred.
• Familiarity with Alzheimer's/dementia care is a plus.
• Proficient with electronic documentation systems.
• Team-oriented and collaborative mindset.

Responsibilities

• Assist residents to reach and maintain independence by helping with activities of daily living and implementing scheduled services.
• Participate with registered nurses in care plan development and reviews; attend care conferences.
• Assist with admission process as directed.
• Safely administer medications and implement treatments as directed.
• Complete documentation with accuracy and attention to detail.
• Mentor and support Resident Assistants through leadership and coaching.
• Manage medication orders, refills, and overall processes.
• Perform supervisory visits and chart reviews; ensure compliance with policies and Medicare license.
• Communicate effectively with residents, families, staff, and other health care providers; maintain confidentiality.
• Participate in agency-wide quality improvement activities and initiatives.
• Follow policies and participate in the organization's quality improvement program.

Schedule & Compensation

What You’ll Do

As the LPN Care Coordinator, you'll play an important part in ensuring residents receive exceptional care each day while working within a supportive interdisciplinary team. Your responsibilities include:

• Assist each client to reach and maintain his or her highest level of performance and independence by performing assistance with activities of daily living and implementing scheduled services as needed
• Participates with registered nurses in the development and review of care plans; participates in care conferences
• Assist with the admission process as directed
• Perform safe administration of medications and implementation of treatments and other therapies as directed
• Completing documentation with accuracy and attention to detail
• Supporting Resident Assistants through mentorship, coaching, and positive leadership Managing medication orders, refills, and overall medication processes
• Performing supervisory visits, completing chart reviews, and ensuring compliance with company policies and our Medicare Comprehensive License
• Demonstrate effective written and verbal communication with residents, family members, staff, and other health care providers. Maintain service excellence and a high level of confidentiality.
• Actively participate in agency-wide activities and its continuous quality improvement
• Understand the importance of seeking assistance as needed and demonstrate dependability and capability of following instructions. Adhere to policies, and procedures and participate in the organization's quality improvement program.

What We're Looking For

• Current Minnesota LPN license
• Strong communication skills with the ability to work professionally and respectfully with staff, residents, and families
• Experience in Assisted Living highly preferred
• Familiarity with Alzheimer's/dementia care is a plus
• Confidence using electronic documentation systems
• A team-oriented mindset and desire to contribute to a supportive work culture
