Job summary

Monarch Healthcare Management is hiring an LPN to provide residents with high‑quality direct routine care under the supervision of the RN and in accordance with Minnesota BON standards. You will participate in plans of care and ensure timely reporting of changes to the RN.

You will supervise unlicensed nursing staff as needed, monitor patient status, and support residents’ physical, mental, and psychosocial well‑being within a team‑oriented setting.

Qualifications

• Completion of MN Board of Nursing approved nursing program.
• Current Minnesota LPN license.
• Current CPR certification.
• Experience in skilled nursing or assisted living facilities.

Responsibilities

• Provide residents with high‑quality direct routine care under RN supervision.
• Assist in developing and modifying the resident Plan of Care.
• Collect health data, report changes to RN, and ensure timely interventions.
• supervise unlicensed staff, clarify expectations, and escalate when needed.
