Job summary

Behavioral Health Network, Inc seeks a Licensed Practical Nurse to join the Transitional Support Services team in Worcester, MA, providing compassionate, person-centered nursing care under the direction of clinical leadership.

You will perform assessments, implement plans, educate patients and families, document thoroughly, and ensure HIPAA and regulatory compliance in a busy behavioral health setting. Massachusetts LPN license and one year of nursing experience preferred.

Qualifications

• Current, unrestricted Massachusetts LPN license.
• At least one year of nursing experience preferred.
• Ability to work in a structured behavioral health environment.
• Capacity to lift up to 25 pounds with or without accommodation.

Responsibilities

• Provide compassionate, patient-centered LPN care under nursing and medical leadership.
• Share health status and treatment responses to aid care planning.
• Administer nursing orders and support individualized care plans.
• Complete health and nursing assessments, progress notes, and documentation.
• Maintain medication safety and records, ensure regulatory compliance.
• Educate patients and families about conditions, treatment, wellness.
• Maintain HIPAA confidentiality and document shift reports.
• Collaborate with medical staff to coordinate services and continuity of care.
