Job summary

Center for Human Development (CHD) seeks a part-time LPN to work 32 hours weekly at the Griswold Center in Palmer, MA. Fridays are required, with flexible hours Monday-Friday.

The LPN will manage clinic triage, medication refill requests, lab reviews, and prior authorizations while collaborating with the care team in our behavioral health outpatient clinic. The successful candidate will have an Associates degree or higher and LPN licensure, with two years of psychiatric experience preferred.

Qualifications

• Two years of psychiatric population experience preferred.
• Valid driver's license and registered vehicle.

Responsibilities

• Develop strong, supportive relationships with clients.
• Implement client medication treatment plans.
• Assess client needs, provide health education, and discuss medications.
• Coordinate clients’ medical treatment with other providers.
• Participate in team communications and CHD policy adherence.
• Attend weekly and psychiatry team meetings.
