Mental Health Association - Western MA is seeking an LPN to support GRIT and DCF young adult programs. The role blends clinical nursing with person-centered care in community residential settings, ensuring safe medication oversight and recovery-focused support.

Schedule is Tue-Sat 9am-5pm, pay rate $33-$35/hour. Applicants must hold a Massachusetts LPN license, have strong care coordination skills, and be comfortable working in a collaborative, multi-disciplinary team.

Qualifications

• Basic knowledge of psychotropic medications, common medical conditions and wellness principles.
• Ability to coordinate care and advocate effectively within the healthcare system.
• Must be at least 18 years old.
• LPN license in Massachusetts is required.
• At least one year of applicable experience is preferred.

Responsabilités

• Assess the physical health needs of participants and help shape individualized care plans.
• Monitor medications, educate participants about treatment, and report side effects.
• Coordinate care with external providers and track paperwork.
• Provide nursing support for the DCF living programs (max 10 hrs/week).
• Contribute to treatment planning, discharge planning, and wellness initiatives.
