Job summary

Valor Healthcare, Inc. is seeking a dedicated Licensed Practical Nurse to join the Community Based Outpatient Clinic (CBOC) in Okeechobee. This weekday position offers competitive pay, excellent benefits, generous time off, and a patient-centered environment serving veterans.

You will work with the PACT team to provide nursing care, coordinate with clinicians, and educate patients on self-management and prevention. A valid LPN/LVN license and VA or related credentials are required.

Qualifications

• Diploma or certificate of an accredited practical nursing program.
• One year of LPN/LVN experience in primary care or ambulatory settings preferred.
• Must be licensed and in good standing with the VA or applicable state agency.

Responsibilities

• Provide prescribed nursing care and personal assistance as directed by clinicians.
• Collaborate with the PACT team and extended care providers to develop patient care plans.
• Administer medications as ordered and document in the electronic medical record.
• Perform routine nursing assessments and patient education for self-management and wellness.
• Maintain patient confidentiality and ensure compliance with VA and clinic policies.
