Mozaic Senior Life is seeking a Program Nurse - LPN for the Adult Day Program in Connecticut. This part-time role (22.5 hours/week) focuses on coordinating nursing and social services for adult day participants.

Minimum qualifications include an accredited nursing degree, CT nursing license, CPR certification, and 2-3 years of nursing experience with elderly populations. Bilingual Spanish preferred. Travel to client homes and community sites may be required.

Job Objective: Coordinates delivery of nursing and social services for adult day participants

Minimum Qualifications

Education: Graduate of accredited nursing school

Experience: 2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.

Licensure: State of Connecticut Nursing License; CPR certification

Bilingual Spanish Prefered
