Mozaic Senior Life is seeking a Program Nurse - LPN for the Adult Day Program in Connecticut. This part-time role (22.5 hours/week) focuses on coordinating nursing and social services for adult day participants.

Qualifications

• Education: Graduate of accredited nursing school.
• Experience: 2-3 years of nursing experience; elderly/dementia experience preferred.
• Licensure: State of Connecticut Nursing License; CPR certification.

Responsibilities

• Conduct new client intakes and assessments to determine referrals and encourage participation.
• Assess nutritional, medical, physical and mental functional needs during initial and ongoing assessments within nursing scope.
