Bristol Adult Resource Center (BARC) in Bristol, CT, is seeking a licensed practical nurse to provide healthcare support for adults with developmental and intellectual disabilities. You will administer medications, assist with nursing tasks, and coordinate with the RN and guardians to address medical needs.

The role is full-time with night shifts: Tue–Thu 10:30pm–8:30am and Fri 11:00pm–8:30am, totaling 39.5 hours weekly.

Qualifications

• High school diploma or equivalent (implied).
• Active LPN/LVN license in Connecticut.
• 1 year of LPN experience preferred.
• Active CPR certification.
• Experience with adults with developmental disabilities preferred.

Responsibilities

• Administers medications according to DDS policies.
• Assists RN with delegated tasks and trainings.
• Collaborate with nursing and support teams and guardians.
• Review medical files to ensure needs are met and compliant.
• Prepare information for required nursing reports and assessments.
• Perform hands-on nursing services and assist with daily activities.
• Maintain confidentiality of medical records and HIPAA information.
• Obtain and maintain required certifications and trainings.
• Perform other duties as assigned.

We are seeking a highly motivated and dynamic Licensed Practical Nurses (LPN) to assist in caring for the health and wellbeing of Individuals with Developmental and Intellectual Disabilities. You will provide a range of services for the individuals based on their needs. This includes but is not limited to routine nursing services, personal care and domestic household services. Requirements

Minimum Requirements:

• High school diploma or equivalent.
• At least age 18.
• Must have an active LPN or LVN state license in Connecticut.
• One year of LPN experience preferred.
• Active CPR certification.
• One year working with adults with developmental disabilities preferred not required.
• Working knowledge of DDS and related training requirements and regulations preferred not required.
• Valid driver’s license, clean driving record and reliable transportation.

Responsibilities

• Administers medications according to DDS policies and procedures. Destroys medications with Program Manager or Supervisor as needed.
• Assists RN with nursing delegated tasks, staff trainings and in-services.
• Collaborate with the nursing team, support teams, and guardians to provide effective care.
• Reviews all individuals’ medical files on a regular basis to ensure all medical needs are being met to comply with all licensing regulations, accreditation criteria, and agency policies.
• Prepares information for required reports for meetings, including annual and quarterly nursing assessments as assigned by the RN.
• Perform hands on nursing services.
• Assists with routine individual care and daily activities.
• Electronically record daily activities maintaining confidentiality and complying with all policies and procedures to protect the identity, medical, HIPPA information.
• Obtain and maintain all required certifications and trainings (in person and online).
• Perform other duties as assigned.

Physical Efforts

• May include extended periods of standing, sitting, walking with occasional periods of stooping, crouching, kneeling.
• Positions may require the ability to independently lift at least 25 pounds, push/pull varying weight (wheelchairs) or have to safely transfer ~35 pounds.
