Job summary

STAR, Inc. is seeking a compassionate LPN to provide dedicated 1:1 nursing support to a young woman with Down Syndrome and Type 1 Diabetes in a community-based day program in Norwalk, CT.

The stable Monday–Friday schedule runs 8:30 a.m. to 3:30 p.m., with no weekends or mandatory overtime. A Connecticut LPN license is required, and a valid driver’s license is preferred to support transportation to activities.

Qualifications

• Current, active Connecticut LPN license required.
• Experience supporting individuals with intellectual and developmental disabilities preferred.
• Valid driverlicense required.

Responsibilities

• Assess, plan, implement, and evaluate the individual's nursing needs during program hours.
• Coordinate daily schedule and activities with Site Manager.
• Support participation in educational, recreational, and community activities aligned to the individual's interests.
• Provide transportation support to local activities as needed.

LPN – 1:1 Nursing Services

$30.00/hour

STAR is a nonprofit organization dedicated to supporting individuals of all ages with intellectual and developmental disabilities, while also providing valuable services and resources to their families throughout lower Fairfield County.

We are seeking a compassionate and energetic LPN to provide dedicated 1:1 nursing support to a wonderful young woman with Down Syndrome and Type 1 Diabetes in a community-based day program. This is a stable Monday-Friday position with no weekends, shift work, or mandatory overtime. The regular schedule is 8:30 a.m. – 3:30 p.m. Our day program is closed on major holidays, providing additional time to enjoy with family and friends.

LPNs of all experience levels and clinical backgrounds are encouraged to apply!

Responsibilities

• Provide nursing care throughout program hours using the nursing process to assess, plan, implement, and evaluate the individual's needs.
• Partner with the Site Manager to coordinate the individual's daily schedule and activities.
• Support participation in educational, recreational, and community-based activities based on the individual's interests, such as art, cooking, yoga, bowling, and more.
• Provide transportation support to and from local community activities as needed.
