Job summary

Easterseals PORT Health is hiring a part-time Licensed Practical Nurse to support adults with intellectual/developmental disabilities and co-occurring behavioral health needs in our ACTH in Woodbridge, VA. The LPN will administer medications, perform nursing assessments, and coordinate with a multidisciplinary team to ensure safe, compliant care.

Qualifications

• Current valid, unrestricted Licensed Practical Nurse license in Virginia.
• Experience in crisis group home or behavioral health settings preferred.
• Strong communication skills, both verbal and written, and good documentation habits.

Responsibilities

• Provide medical support and nursing services to adults with IDD and co-occurring behavioral health needs.
• Administer medications and conduct nursing assessments.
• Maintain accurate health records and collaborate with interdisciplinary teams.
• Support residents' health and wellness within the crisis therapeutic home.

At Easterseals PORT Health, we’re seeking a compassionate and dedicated LPN to join our Virginia REACH team designed to provide support to individuals in our Adult Crisis Therapeutic Home (ACTH) located in Woodbridge, VA.

• $5,000 sign-on and retention bonus*

Schedule

Part-Time – Saturday and Sunday 7:00am – 7:00pm
Part-Time – Saturday and Sunday 7:00pm – 7:00am

What You’ll Do

The Licensed Practical Nurse (LPN) provides medical support and nursing services for individuals with intellectual/developmental disabilities and co-occurring behavioral health needs within our adult crisis therapeutic home. Responsibilities include administering medications, conducting nursing assessments, providing health education, and ensuring compliance with regulatory standards. This role involves collaboration with interdisciplinary teams, maintaining accurate documentation, and supporting individuals’ overall health and wellness.

What We’re Looking For

To join our team, you must be able to provide care in a crisis group home, have a passion for helping others, experience with MS Office suite and data entry, strong verbal and written communication skills, as well as a valid driver’s license and car insurance. We also require the following:

• Current valid, unrestricted licensed as a Practical Nurse in state of Virginia
• Knowledge of the MH and I/DD systems of care preferred
• Experience with crisis prevention and intervention services preferred
