Position Summary

Department Summary

Memorial 4 North is a 24 bed, mixed acuity, unit consisting of telemetry and medical/surgical patients. The unit provides assessment, care, and treatment of acutely ill patients from ages 18 and above (unless otherwise indicated). The unit operates 24 hours per day, seven days per week. Each of the 24 private rooms has the capability of telemetry monitoring.

Common Candidates For Admission Include
  • Patients with diagnosis of Transient Ischemic Attack, Cerebrovascular Accident, and Seizures.
  • Patients requiring telemetry monitoring.
  • Patients requiring inotropic therapy for cardiac or renal disease.
Qualifications
  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.
  • IV Certification, preferred.
Department Qualifications
  • Obtain and maintain current American Heart Association (AHA) training in National Institute's of Health Stroke Scale (NIHSS) is required within 90 days of hire.
  • Certified Nurse in Critical Care (CNCC) certification must be obtained within six months of hire and maintained in accordance with organizational standards. Training classes are sponsored by the Educational and Development Department.
  • Obtain and maintain Bedside Swallow Screening competency.
  • Complete required orientation for position.

  • Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients’ responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse.
  • Collects specimens as ordered.
  • Interacts with and contributes to the professional development of peers, colleagues, and students.
  • Collaborates with patient, family, and others in the conduct of nursing practice.
  • LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction.
  • Attains knowledge and competency that reflects current nursing practice and applicable policies.
  • Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules and regulations, during nursing practice.
  • Exhibits High Reliability Organization (HRO) tools and tones.
  • Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.
Working Conditions
  • Meal and break times are coordinated with other staff members to maintain adequate coverage.
  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.
  • Work weekends and holidays as assigned or as applicable based on department needs.
  • Cross training depending upon department needs.
