Job summary

Resthaven, a faith-based not-for-profit in Holland, Michigan, is seeking an LPN/RN to provide direct nursing care, administer medications, and monitor residents in a senior care setting. The role supports CNAs, maintains accurate EHRs, and adheres to infection control protocols in a dynamic long-term care environment.

The position offers full-time, part-time, and PRN options with shifts including nights. A sign-on bonus is available for full-time applicants, and there is tuition reimbursement

Qualifications

• High school diploma or GED required.
• Graduate of an accredited LPN/RN nursing program.
• Successful completion of State Board of Nursing licensure.
• Prior LPN/RN experience preferred in long-term care or geriatric settings.
• Maintains continuing education to keep licensure active.

Responsibilities

• Provide direct nursing care and implement resident care plans.
• Administer medications and treatments accurately.
• Monitor health status, vital signs, wounds, and skin integrity.
• Document and report changes in resident condition promptly.
• Maintain EHR documentation and charting.
• Ensure infection control and regulatory compliance.
• Lead and support CNAs and care staff.
• Float between units to cover staffing needs.

Essential Responsibilities - LPN/RN (Licensed Practical Nurse / Registered Nurse)

• Provide direct nursing care and implement individualized resident care plans
• Administer medications, treatments, and pain management interventions safely and accurately
• Monitor resident health status, including assessments, skin integrity, wound care, and vital signs
• Observe, assess, document, and promptly report changes in resident condition
• Execute physician orders accurately and within required timeframes
• Maintain accurate electronic health record (EHR) documentation and clinical charting
• Ensure compliance with infection control protocols and all State and Federal healthcare regulations
• Respond effectively to urgent and emergent resident care situations using sound clinical judgment
• Communicate clearly and professionally with residents, families, physicians, and interdisciplinary care teams
• Lead, delegate, and support CNA and clinical support staff while fostering teamwork and accountability
• Assist with staffing coverage and provide unit support as needed
• Deliver comprehensive shift reports to ensure continuity of care
• Provide exceptional resident-centered customer service by exceeding resident and family expectations
• Float between units based on operational and resident care needs

Education, Training, Experience

• High school diploma or GED required
• Graduate of an accredited Licensed Practical Nurse (LPN) or Registered Nurse (RN) or nursing program
• Successful completion of State Board of Nursing licensure examination
• Previous LPN/RN nursing experience preferred, ideally in long-term care, skilled nursing, rehabilitation, assisted living, or senior care.
• Experience working with geriatric or elderly populations preferred
• Must maintain all continuing education requirements to keep nursing licensure active and in good standing
• Participates in ongoing learning, professional development, and required in-service training

Required Licenses, Registration, And/or Certifications

• Current, active Michigan Board of Nursing License as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
