PACS Group is seeking a 2nd shift LPN in Perrysburg, OH, offering starting pay of $32/hour with a shift differential and a 3:00 PM to 11:30 PM schedule. You will lead the evening unit, supervise CNAs, and coordinate with families and physicians to ensure high-quality resident care.

You’ll bring an Ohio LPN license, CPR, and strong leadership. This role emphasizes compassionate care for seniors, infection control, and collaborative teamwork within a supportive post-acute setting.

Qualifications

• Current, unencumbered Ohio LPN license in good standing.

Three Meadows Post Acute in Perrysburg is hiring a 2nd shift LPN to join a supportive team that puts residents first.

On second shift, you set the tone for the evening: guiding your CNAs, caring for residents, and keeping the unit running smoothly.

What You’ll Do

• Provide hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring changes in condition
• Lead your unit’s evening shift by supervising and supporting nursing assistants and making daily rounds
• Be the link between residents, families, and physicians, including physician orders, family updates, and care coordination
• Keep care plans on track, working with the care plan team and contributing to quality assurance
• Help new residents settle in, from admission and orientation to discharge planning
• Promote a safe, infection-free environment and help staff follow safety and infection control practices

What You’ll Bring

• A graduate of an approved LPN program
• Current, unencumbered Ohio LPN license in good standing with the State Board of Nursing
• Current CPR certification
• Leadership skills and a team-first attitude
• Patience, tact, and compassion for working with older adults and their families
• Ability to lift 50 lbs and move through the shift comfortably
• PointClickCare (PCC) experience is a plus, but not required
