Job summary

Three Meadows Post Acute in Perrysburg, OH is hiring a 2nd shift LPN to join a supportive team that puts residents first. On the 3:00 PM to 11:30 PM shift, you’ll provide hands-on nursing care and lead a team of CNAs to keep the unit running smoothly.

You will coordinate care with physicians, update care plans, and help new residents settle in while maintaining a safe, infection-free environment. Ohio LPN license and CPR are required, PCC experience is a plus.

Qualifications

• Graduate of an approved LPN program.
• Current Ohio LPN license in good standing.
• Current CPR certification.
• Leadership and a team-first attitude.
• Ability to lift 50 lbs.
• PCC experience is a plus.

Responsibilities

• Provide hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring changes in condition.
• Lead your unit’s evening shift by supervising and supporting nursing assistants and making daily rounds.
• Be the link between residents, families, and physicians, including orders and care coordination.
• Keep care plans on track, collaborate with the care plan team, and contribute to quality assurance.
• Help new residents settle in, from admission to discharge planning.
• Promote a safe, infection-free environment and ensure safety practices.

On second shift, you set the tone for the evening: guiding your CNAs, caring for residents, and keeping the unit running smoothly.

What You’ll Do

• Provide hands-on nursing care to residents, including medication administration, treatments, wound care, and monitoring changes in condition
• Lead your unit’s evening shift by supervising and supporting nursing assistants and making daily rounds
• Be the link between residents, families, and physicians , including physician orders, family updates, and care coordination
• Keep care plans on track , working with the care plan team and contributing to quality assurance
• Help new residents settle in , from admission and orientation to discharge planning
• Promote a safe, infection-free environment and help staff follow safety and infection control practices

What You’ll Bring

• A graduate of an approved LPN program
• Current, unencumbered Ohio LPN license in good standing with the State Board of Nursing
• Current CPR certification
• Leadership skills and a team-first attitude
• Patience, tact, and compassion for working with older adults and their families
• Ability to lift 50 lbs and move through the shift comfortably
• PointClickCare (PCC) experience is a plus, but not required
