Columbiana Health & Rehabilitation in Columbiana, AL is seeking a Licensed Practical Nurse (LPN) for the day shift on a 12-hour schedule (7a–7p). This full-time role offers $25.00–$31.00/hour based on experience and a $10,000 sign-on bonus.

You will provide resident-centered care and work within a close-knit nursing team. You’ll administer meds, monitor condition, document care, and collaborate with RNs, physicians, and CNAs.

Qualifications

• Current Alabama Licensed Practical Nurse (LPN) license in good standing.
• Strong clinical and communication skills.
• Compassionate, resident-centered approach.
• Ability to work effectively as part of a nursing team.
• Hospital, rehabilitation, skilled nursing, or long-term care experience preferred.
• LPNs encouraged to apply if coming from larger healthcare environments.

Responsibilities

• Provide compassionate, resident-centered nursing care.
• Administer medications and treatments according to physician orders.
• Monitor residents and promptly report changes in condition.
• Perform nursing procedures within the LPN scope of practice.
• Communicate with RNs, physicians, and other healthcare providers.
• Supervise, mentor, and support CNAs.
• Collaborate with interdisciplinary team members.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with residents and families.
• Promote resident safety, dignity, comfort, and quality of life.
