Job summary

Columbiana Health & Rehabilitation is seeking an experienced Licensed Practical Nurse (LPN) for the day shift on a 12-hour schedule. You will provide compassionate resident-centered care, administer medications, monitor changes, and collaborate with the nursing team to ensure high-quality outcomes.

Qualifications

• Must hold Alabama LPN license in good standing.
• Strong clinical and communication skills.
• Compassionate, resident-centered approach.
• Ability to work effectively as part of a nursing team.
• Experience in hospital/rehabilitation/long-term care preferred.

Responsibilities

• Provide resident-centered nursing care.
• Administer medications and treatments per physician orders.
• Monitor residents and report changes promptly.
• Perform nursing procedures within LPN scope of practice.
• Communicate with RNs, physicians and other healthcare providers.
• Supervise, mentor, and support CNAs.
• Collaborate with interdisciplinary team members.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with residents and families.
• Promote resident safety, dignity, comfort and quality of life.
