Job summary

Paycom - ATS is seeking a Licensed Practical Nurse to perform scheduled home health visits in Yukon, OK and surrounding areas. You will conduct head-to-toe assessments, administer medications as ordered, and document care in the electronic chart.

Candidates must hold an Oklahoma LPN license, have at least 1 year in home health or acute care, and be able to lift 20 lbs. You must own a vehicle with insurance and have a phone for after-hours calls; on-call rotations and weekly chart updates are

Qualifications

• Active LPN license in Oklahoma.
• At least 1 year experience in home health or acute-care.
• Experience with elderly, middle-aged and/or pediatric patients.
• Ability to lift 20 lbs.
• Own a vehicle with current auto insurance.
• Telephone available for after-hours calls.

Responsibilities

• Complete scheduled home health visits (30–60 minutes) with comprehensive assessments.
• Perform psycho-social, pain, spiritual assessments; monitor vitals; administer meds as ordered and provide treatments.
• Submit documentation electronically within 24 hours of visits.
• Attend weekly case conferences and relevant staff meetings.
• Perform PRN visits as necessary.
• Review and update patient records regularly.
• Update medication profiles and conferencing docs weekly.
• Return documentation for corrections within 24 hours.
• Join on-call rotation to ensure 24-hour nursing coverage.

Job description

1. Complete scheduled home health visits by coordinating working schedule to fit patient's time and needs.2. All visits are to be minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, psycho-social assessment, pain assessment, spiritual assessment, vital signs, administer medications and give treatments as order by physician.3. All patient visit documentation must be completed and submitted electronically no later than 24 hours from time patient visit was completed.4. Attend weekly case conference meetings and any staff meetings upon notification.5. PRN visits to be made as necessary.6. Regularly review and update patient records as they pertain to the care of each patient.7. Complete medication profile updates and case conference documentation no less than weekly.8. All documentation returned for corrections, must be corrected and resubmitted within 24 hours of return.9. Will participate in an on-call rotation to ensure 24 hours nursing care coverage to patients.Current Licensed Practical Nursing license approved by the Oklahoma Nursing Board of Regents without restrictions.At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged and/or pediatric care.Able to lift a minimum of 20 lbs without difficulty.Must own a vehicle in which to make visits and maintain current automobile insurance coverage.Must have a telephone available for after-hours calls when needed.
