Job summary

Loving-Care-in-Home-Health-Services is seeking an RN supervisor to oversee patient care across assigned cases, coordinating nursing staff, home health aides, social work and therapies as ordered by physicians.

You will conduct total head-to-toe assessments during home visits, ensure timely documentation, and develop plans of care with communication to physicians. Travel to patient homes is required, with after-hours calls as needed.

Qualifications

• Must have current Registered Nursing license approved by the Oklahoma Nursing Board of Regents without restrictions.
• At least one year experience in either home health setting or acute-care setting with elderly, middle aged, and/or pediatric care.
• Must be able to lift a minimum of 20 lbs without difficulty.
• Must own a vehicle in which to make home visits and maintain current automobile insurance coverage.
• Must have a telephone available for after-hours calls when needed.

Responsibilities

• Provide supportive care and therapies per attending physician orders and interdisciplinary plan of care.
• Coordinate work schedule to fit patient needs; spend 30 minutes to 1 hour per home visit for assessments and treatments.
• Complete and submit all patient visit documentation electronically within 24 hours of visits.
• Develop Plan of Care with orders and communications to physician.
• Conduct PRN visits as needed.

Job description

1. Supervise and manage all care given to assigned case management patients. Responsible for identifying patient and/or family needs and overseeing all patient care management for assigned patients, to include nursing staff, home health aides, social work and all therapies that are assigned to patients as ordered by physician.2. Responsible for providing supportive care and appropriate services in accordance with attending physician's orders and the interdisciplinary team plan of care.3. Coordinate work schedule to fit patient's time and needs. Spend minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, vital signs, administer medications as ordered, complete supervisory of patient care assessments and give treatments as ordered by physician.4. All patient visit documentation must be completed and submitted electronically no later than 24 hours of the time patients visits are completed.5. Develop Plan of Care with all orders and communication going to patient's physician.6.
