Job summary

Loving Care In-Home Health Services is seeking an experienced LPN to join our Yukon, OK home health team. You will perform scheduled visits, conduct comprehensive assessments, and manage patient documentation with timely electronic submissions.

Requirements include an active OK LPN license, 1 year of home health or acute care experience, ability to lift 20 lbs, vehicle ownership, and a phone for after-hours calls. This is a full-time, day shift role in the Yukon area.

Qualifications

• Must hold a current LPN license approved by the Oklahoma Nursing Board without restrictions.
• At least 1 year of experience in home health or acute care, caring for elderly, middle-aged and/or pediatric patients.
• Ability to lift at least 20 lbs.
• Must own a vehicle and maintain current auto insurance.
• Must have a phone for after-hours calls.

Responsibilities

• Complete scheduled home health visits, coordinating times to fit patient needs.
• Conduct comprehensive head-to-toe assessments and document results.
• Submit all patient visit documentation electronically within 24 hours.
• Attend weekly case conferences and staff meetings.
• Provide PRN visits as needed.
• Maintain and update patient records regularly.
• Update medication profiles and conference documentation weekly.
• Correct any documentation errors within 24 hours of notification.
• Participate in on-call rotation to ensure 24-hour nursing coverage.

Responsibilities

1. Complete scheduled home health visits by coordinating working schedule to fit patient's time and needs.
2. All visits are to be minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, psycho-social assessment, pain assessment, spiritual assessment, vital signs, administer medications and give treatments as order by physician.
3. All patient visit documentation must be completed and submitted electronically no later than 24 hours from time patient visit was completed.
4. Attend weekly case conference meetings and any staff meetings upon notification.
5. PRN visits to be made as necessary.
6. Regularly review and update patient records as they pertain to the care of each patient.
7. Complete medication profile updates and case conference documentation no less than weekly.
8. All documentation returned for corrections, must be corrected and resubmitted within 24 hours of return.
9. Will participate in an on-call rotation to ensure 24 hours nursing care coverage to patients.

Qualifications

• Current Licensed Practical Nursing license approved by the Oklahoma Nursing Board of Regents without restrictions.
• At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged and/or pediatric care.
• Able to lift a minimum of 20 lbs without difficulty.
• Must own a vehicle in which to make visits and maintain current automobile insurance coverage.
• Must have a telephone available for after-hours calls when needed.
