Job summary

Loving-Care-in-Home-Health-Services is seeking a dedicated Licensed Practical Nurse (LPN) to provide in-home health visits. The role focuses on comprehensive assessments, medication administration as ordered by physicians, and psychosocial support across patient populations.

Applicants must hold an Oklahoma LPN license, have at least one year of home health or acute-care experience with elderly or pediatric care, and be able to lift 20 lbs.

Qualifications

• Ability to complete total head-to-toe assessments and various nursing tasks during visits.
• Accurate and timely electronic documentation within 24 hours of each visit.
• On-call rotation participation to ensure 24-hour nursing coverage.

Responsibilities

• Complete scheduled home health visits with 30 minutes to 1 hour per patient.
• Perform psycho-social, pain, spiritual, and vital sign assessments; administer meds as ordered.
• Review and update patient records; complete medication profiles weekly; submit documentation within 24 hours.
• Attend weekly case conferences and staff meetings; participate in PRN visits as needed.
• Maintain driving schedule and vehicle insurance; have a telephone for after-hours calls.

Job description

1. Complete scheduled home health visits by coordinating working schedule to fit patient's time and needs.2. All visits are to be minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, psycho-social assessment, pain assessment, spiritual assessment, vital signs, administer medications and give treatments as order by physician.3. All patient visit documentation must be completed and submitted electronically no later than 24 hours from time patient visit was completed.4. Attend weekly case conference meetings and any staff meetings upon notification.5. PRN visits to be made as necessary.6. Regularly review and update patient records as they pertain to the care of each patient.7. Complete medication profile updates and case conference documentation no less than weekly.8. All documentation returned for corrections, must be corrected and resubmitted within 24 hours of return.9. Will participate in an on-call rotation to ensure 24 hours nursing care coverage to patients.Current Licensed Practical nurse license approved by the Oklahoma Nursing Board of Regents.At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged, and/or pediatric care.Able to lift a minimum of 20 lbs without difficulty.Must own a vehicle in which to make visits and maintain current automobile insurance coverage.Must have a telephone available for after-hours calls when needed.
