Job summary

The Fountains at Atco is seeking a dedicated LPN to join our compassionate care team in a continuing care retirement community. You will provide high-quality nursing care, administer medications, conduct assessments, and help develop individualized care plans to support residents’ well-being.

This on-site role offers 12-hour shifts (7a-7p and 7p-7a) and opportunities to work closely with an interdisciplinary team, ensuring residents thrive in a supportive, welcoming environment.

Qualifications

• Current LPN licensure in New Jersey.
• Minimum 1 year of experience in a long-term care or healthcare setting.
• Excellent communication and interpersonal skills.
• Ability to thrive in a fast-paced environment.

Responsibilities

• Conduct initial and ongoing assessments of residents' physical and emotional needs.

• Monitor residents' conditions and report changes to the charge nurse or other healthcare professionals.
• Maintain accurate and detailed records of resident care and services provided.
• Collaborate with other healthcare professionals to ensure seamless transitions and coordination of care.
