Job summary

The Fountains at Atco is seeking a dedicated Licensed Practical Nurse to join our care team. You will deliver high-quality nursing care to residents, administer medications, and support individualized care plans in a warm, resident-centered community.

Qualifications

• Current LPN licensure in New Jersey.
• Minimum 1 year of experience in a long-term care or healthcare setting.
• Strong communication and interpersonal skills.

• Conduct initial and ongoing assessments of residents' physical and emotional needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team.
• Monitor residents' conditions and report changes to the charge nurse or other healthcare professionals.
• Maintain accurate and detailed records of resident care and services provided.
• Collaborate with other healthcare professionals to ensure seamless transitions and coordination of care.
