Job summary

The Gables of Westerville is seeking a Licensed Practical Nurse (LPN) for a full-time night shift (7pm–7:30am) in Westerville, OH. The role supports resident care, contributes to a collaborative team, and aligns with corporate service values and resident welfare.

Responsibilities include charting notes, managing physician orders, coordinating appointments, and maintaining resident charts while ensuring compliance with policies, regulations, and safety standards.

Qualifications

• Must hold an active nursing license in good standing.
• Thorough working knowledge of current care standards and regulations.
• Ability to supervise care staff in the absence of leadership.
• Knowledge of current medication regulations and laws.
• Ability to communicate with physicians, pharmacies, families, and community staff.

• Assist in oversight of Wellness staff when leadership is absent.
• Ensure high-quality care while adhering to policy, regulation and law.
• Chart resident notes and review end-of-shift notes.
• Record new or changed physician orders and maintain charts.
• Develop and maintain relationships with physicians and healthcare providers.
• Conduct resident assessments and appraisals as per protocol.
• Respond to medical emergencies and monitor changes in residents.
• Schedule medical appointments and maintain calendars.

The Gables of Westerville is looking for Licensed Practical Nurses (LPNS).

Full time Nightshift 7pm - 7:30am

Licensed Practical Nurse (LPN) Duties:

• Present a professional demeanor that communicates a corporate philosophy of service, goodwill, and genuine interest, dignity, and respect, celebration and choice in each resident's needs.
• Assists in the oversight of all Wellness staff in the absence of the Director of Nursing, Wellness Director and Care Coordinator.
• Ensure outstanding, appropriate care is delivered while adhering to community policy, regulation, and law.
• Chart resident notes appropriately.
• Review end of shift notes.
• Record new/changed physician orders.
• Maintain healthcare provider relations.
• Perform resident assessments according to established protocol.
• Conduct appraisals of resident needs.
• Respond to medical emergencies.
• Monitor and record physical, mental, and emotional changes observed in residents.
• Maintain current and accurate resident charts.
• Assist in scheduling medical appointments and maintain a current medical appointment calendar.
• Develop and maintain positive working relationships with physicians.
• Participate in community surveys.
• Meet with families/residents during the admissions process.
• Ensure rooms are ready for new residents.
• Report any suspicion or evidence of elder abuse, mistreatment or neglect as outlined in Personnel Policies.
• Perform other duties as assigned.

Requirements

• Must have current nursing license in good standing.
• Thorough working knowledge of current care standards and regulation.
• Ability to maintain and update effective service plans.
• Ability to supervise care staff.
• Comprehensive working knowledge of current medication regulation and law.
• Knowledge of the requirements for providing care and supervision appropriate to the residents.
• Ability to communicate with physicians, pharmacies, families, and community staff.
• Be in good health, and physically mental and capable of performing assigned tasks. Good physical health shall be verified by a health screening performed by a physician not more than 30 days prior to employment.
• Demonstrate freedom from pulmonary tuberculosis no more than 30 days before employment.
• Must be criminally cleared by DOJ and FBI prior to the initial presence in the facility.
