Job summary

Vernon Health and Living in Wabash, IN is seeking a dedicated Licensed Practical Nurse to provide direct nursing care to residents under RN supervision, manage medication administration, and support comfort and well-being.

You will perform wound, ostomy, and catheter care, monitor vital signs, document care, and collaborate with CNAs and families. Flexible shifts and a team‑driven environment are offered.

Qualifications

• Current LPN license in state of practice.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR certification.
• Long‑term care or geriatric nursing experience preferred.

Responsibilities

• Administer oral, topical, and injectable medications per physician orders.
• Monitor residents for changes in condition and report to the supervising RN.
• Perform wound care, catheter care, ostomy care, and treatments.
• Obtain and document vital signs, weights, and intake and output.
• Implement care plans as directed by the RN and care team.
• Supervise and delegate tasks to CNAs on your shift.
• Communicate with residents, families, and the nursing team.
• Document care accurately and timely in the electronic health record.
• Follow infection control, safety, and facility compliance standards.
