Susquehanna Nursing & Rehabilitation Center in Johnson City, NY is seeking a Licensed Practical Nurse (LPN) to join our nursing team. You will provide high-quality nursing care, administer medications and treatments, and support resident well-being in a team-focused environment.

The position offers an evening shift with both full-time and part-time options. Competitive hourly rate of $29–$32.60 plus a sign-on bonus up to $5,000, reflecting experience and qualifications.

Qualifications

• High School Diploma or equivalent required; Associate's Degree preferred.
• Valid NYS Licensed Practical Nurse (LPN) License required.
• Current CPR Certification required.
• Experience in a skilled nursing facility or with the geriatric population preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of medication administration, infection control, and resident care practices.
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team.
• Compassionate, resident-centered approach to care.

Responsibilities

• Provide nursing care per resident assessments and care plans.
• Administer medications, treatments, enteral feedings, and IV therapies within LPN scope.
• Monitor resident conditions and communicate changes promptly.
• Document care, treatments, observations, and medications accurately.
• Supervise Nursing Assistants to ensure quality resident care.
• Maintain up-to-date resident care information and communicate during shifts.
• Follow infection control and confidentiality standards.
• Participate in interdisciplinary care planning and quality improvement.
• Assist with admissions, transfers, and other nursing responsibilities.

Requirements
